Psoriasis is also a chronic and recurrent disease but typically is not characterized by itching. Psoriasis can vary in severity from one or two lesions to a widespread dermatosis with disabling arthritis or exfoliation. The cause is unknown but the thick scaling is probably due to increased epidermal cell proliferation. A family history of psoriasis is common and usually reflects an autosomal dominant inheritance.
The onset of this disease is somewhat gradual and can vary widely in its severity. The typical course is one of chronic remissions and recurrences that vary in frequency and duration. Factors precipitating psoriatic eruptions include local trauma to the skin, severe sunburn, irritation, topical medications, and withdrawal of systemic corticosteroids.
The lesions are clearly defined and may have papules or plaques with shiny silver scales. Psoriasis characteristically involves the scalp and behind the ears, elbows and knees, the back and buttocks. The nails, eyebrows, axillas, umbilicus and anogenital region may also be affected.
Psoriasis typically affects the scalp, extensor areas of the arms and legs, as well as the back and buttocks area. The lesions are sharply demarcated, usually nonpruritic, erythematous papules. The lesions heal without scarring, and hair growth is not altered. The papules sometimes extend and coalesce, producting large plaques in interesting patterns. Nail involvement may resemble a fungal infection with stippling, pitting, discoloration and debris under the nail plate.
